Patient Scheduling with the Clinical Research Center

To schedule a patient visit, email a request to crc-support@bcm.edu.

The following documents are needed to process scheduling:

• BSLMC-CRC Subject Registration Form
• BCM-CRC Subject Registration Form
• Signed Physician Order
• Signed Consent Form
• Signed Eligibility Checklist (not applicable for screening visit)
• Any other documents as applicable for the visit
