Our Community Health Program exemplifies Baylor's commitment to being a force for health in the community and establishing a patient-centered medical home for our patients.

Our faculty group partners with community-based organizations to provide high quality, evidence-based care to underserved areas and to help promote wellness initiatives in neighborhoods near our community health centers. We seek to develop best practices and new innovative models of primary care delivery.

Harris Health System has partnered with Baylor College of Medicine to provide essential medical services to both adults and children at community health centers throughout Greater Houston.

In addition to providing adult and pediatric primary care, these centers may also offer specialty care, convenient on-site laboratory and pharmacy services combined with nutrition and health education, counseling and other social services to meet each patient's unique needs.

With a fully integrated electronic medical record, the partnership provides residents with a medical home in their neighborhood and coordinated access to higher levels of care when needed.

Same Day Clinics

Through the partnership with Harris Health System, Baylor College of Medicine physicians and advanced practice clinicians treat patients for minor illnesses and injuries at these same day clinic locations:

Pediatric & Adolescent Health Center

Harris Health System's Pediatric and Adolescent Health Center with Baylor College of Medicine physician faculty care for newborns to teens up to 18 years of age.

Thomas Street Health Center: HIV/AIDS Clinic

Content

Harris Health System's Thomas Street Health Center is a freestanding HIV/AIDS clinic, which provides medical care and psychological and social services to those who have been diagnosed with HIV/AIDS. Physicians from Baylor College of Medicine and UTHealth provide primary and specialty care services.

Thomas Street Clinic is staffed by physicians from various Baylor College of Medicine departments.

Ben Taub Hospital

Content

Ben Taub Hospital, the central clinical care facility of the Harris Health System, serves as a teaching hospital for all specialties and their subspecialties. The hospital's chief of staff and the chiefs of every medical and surgical service are full-time Baylor faculty members. 

As the largest hospital in the Harris Health System, Ben Taub provides an impressive range of services, from outpatient general medicine and pediatric clinics, to the most complicated surgical procedures.

Quentin Mease Hospital

Quentin Mease Hospital is a 49-bed hospital located five miles east of the Texas Medical Center. Faculty, residents and fellows of Baylor staff this hospital. Baylor physical medicine and rehabilitation and geriatric trainees see patients at this facility.
