To schedule an appointment with one of our physicians, see phone numbers for clinical services or visit the specialty webpage with which you wish to make an appointment.

  • Please have your insurance card ready when you call.
  • Please arrive on time for your appointment.

This helps us provide timely care to all our patients. If you are not able to make your scheduled appointment, please call in advance to cancel.

Planning for Your Appointment

Please plan to arrive 15 minutes before your appointment time to complete the registration process. Remember to bring your:

  • Photo ID
  • Insurance card(s)
  • Medical records from other physicians (if applicable)
  • Current medications including dosage and frequency
  • Referring physician contact information (if applicable)
  • Preferred pharmacy contact information
  • Emergency contact information

Check-In
Upon arrival, the front desk staff will review your information, provide you with any forms that need to be completed, collect any payment due, and inform the medical staff of your arrival.

Visitor Guidelines
Baylor Medicine does not require most people to wear a mask while in our clinics.  Patients may choose to wear a mask, or request that staff or physicians wear a mask while in the exam room.

Patients and visitors with a cough or other respiratory illness, or who have COVID-19 disease, must wear a mask. If you do not have a mask, one will be provided for you. If you have COVID-19 disease or symptoms on the day of your visit, please alert the front desk upon your arrival.

For all visitors to Baylor Medicine outpatient clinic facilities, we ask that visitors adhere to the following:

  1. It is the parent’s responsibility to be able to appropriately supervise a minor if they are present during a visit. Minors requiring supervision must have another adult present to stay with the child in the waiting room if the visit requires privacy or a procedure is being performed.  
  2. When you arrive to your appointment, and have successfully completed ePreCheckin, please let our front desk staff know that you have arrived to fully complete the check-in process.
  3. If patients are not feeling well, they should notify the front desk upon arrival to the clinic.
  4. If visitors are not feeling well, they should not come to the appointment in person. They can ask to join the appointment by phone.

Timing
We would like to accommodate all of our patients in a timely manner. Please note, if you arrive more than 20 minutes after you appointment time, we may ask if you prefer to wait or reschedule for another day.

We understand that your time is valuable, and we will make every effort to see you at your scheduled appointment time. Unfortunately, emergencies, complications and other issues might lead to unexpected delays.

Baylor Medicine MyChart
We know that having convenient and timely access to your medical records is essential. That’s why we use Baylor Medicine MyChart, a safe and secure means of accessing your medical records and handling routine healthcare needs.

MyChart is available at mychart.bcm.edu.

During your visit, you will be given instructions on how to activate your MyChart account. This private, self-service system allows you to request appointments, review lab results, and message your physician’s medical staff with non-urgent questions. MyChart messages will be answered within two business days and are considered non-urgent.

For any urgent concern, please phone the office directly.

Privacy
Your privacy is of the utmost importance to us, and confidentiality is something we take very seriously. All discussions will be kept between you, your physician, and your care team.

Diagnostic Services
During your appointment, your physician may order laboratory or radiology (imaging) tests such as CT scans, ultrasounds or x-rays to be performed. If you have a diagnostic service provider who you prefer to use, please discuss with your physician during your visit. More information regarding diagnostic services can be found below.

After Visit Summary
An after visit summary will be provided to you or made available on your MyChart account. The summary includes any orders, instructions or follow-up appointments.

Billing
If you have any questions about your Baylor Medicine medical bill, our billing representatives are available from 8 a.m. to 5 p.m., Monday through Friday, to answer any questions you might have. A helpful billing representative can be reached by calling (713) 798-1900.

Feedback
Your feedback is vital to improving our ongoing efforts to provide you with the best care possible. Comment cards are located throughout our clinics so you can provide feedback on our services. In addition, you may receive a Press Ganey patient satisfaction survey in the mail or as an email regarding your visit. Please take a moment to complete the survey.

Diagnostic Services: Labs and Imaging

If your physician has ordered laboratory or radiology (imaging) tests, you can have these completed at one of our Diagnostic Services centers. For laboratory tests and certain imaging tests, no appointment is needed.

General Patient Guidelines

Medication Refills
The fastest way to secure your medication refill may be through your pharmacy. Check with your preferred pharmacy to determine if you have additional refills available to you.

If you require a medication refill from your provider, requests will be reviewed and responded to by the end of the next business day. Please note that certain classes of medications may require authorization from your insurance provider or have additional delays associated with them. Discuss any special requirements for refills with your provider to plan appropriately.

Phone Calls
Non-urgent calls will be returned by the end of next business day. Urgent calls will be returned on the same business day in the order they are received. After-hours calls will be handled efficiently and effectively by our answering service or an on-call physician.

Forms
Forms received by the clinical staff will be processed and returned within 10 business days.
