Baylor Medicine’s Adult Genetics clinic is caring for our patients both in-person and virtually through telehealth phone and video visits.  If you are interested in switching your appointment to a telehealth visit, please contact us using MyChart or call our office at 713-798-7820.


New Adult Genetics Patient Instructions


Step 1: Register with Baylor Medicine

All patients new to Baylor Medicine must be registered before an appointment can be scheduled. The scheduling team will ask you to provide basic personal information and details regarding your diagnosis and insurance or other payment plans.

2. Activate MyChart

Please activate MyChart. A code is needed to activate MyChart. If you have not received a MyChart activation code you can go to, select the Sign Up Now button and then the Request Online button.

MyChart offers patients personalized and secure online access to portions of their medical records. It enables you to securely use the Internet to help manage and receive information about your health.

3. Complete Intake Paperwork

All patients must complete the proper forms found in Adult Genetics Intake Packet the before they can be scheduled for an appointment. This allows us to schedule your appointment with the most appropriate physician. It also may help the providers prepare for your visit.

All patients must complete the General Intake form, the Family History form, the Health Care Providers form, the Patient Question List and Alternative Communications and Designated Contacts form. Please send the requested records, if available.

Based on your responses on the intake forms, medical records may be needed to ensure that you receive optimal care. You can speed up the scheduling process by sending the results of any prior genetic testing (your own or a family member’s) or including the reports wherever indicated on the intake form. Otherwise, you will be informed by the clinic if you need to provide any additional records prior to scheduling.

4. Complete Patient Privacy Form

We take our patients' privacy very seriously. If a family member contacts us for information regarding your genetic diagnosis/medical care, and they are not listed on the Authorization for Release of Information form we will be unable to discuss your information with them until you provide permission.

5. Designate MyChart Proxy

If you are interested in designating someone to have official authorized access to the MyChart portal proxy to your MyChart account, please let the front desk personnel know when you arrive for your next visit.

6. Send the Forms

We have three methods you can use to submit your completed forms:

  • Send as an attachment in a MyChart message at

  • Fax the forms: (713) 798-6450

Upon review of your materials you will be contacted by the Genetics Clinic. Also note that in an effort to be serve all patients, we are unable to offer an appointment to individuals who have not submitted all forms and signed up for MyChart.

At the Appointment

  • We are located at 7200 Cambridge Street, Suite 9A Houston, TX 77030
  • Please plan to arrive about 15 minutes prior to your scheduled appointment.
  • Upon arrival, please check in with the front desk. You will be asked for your photo identification and insurance card.
  • For the convenience of all our patients, individuals who arrive 20 minutes or more late will need to reschedule. This helps to keep our clinic running on time.
  • Generally, patients do not need to fast before their appointment. While genetic testing, when appropriate, is often ordered the day of the appointment, genetic testing does not require fasting.

During your visit, we may determine if genetic testing is needed based on review of your medical history and physical examination findings. We cannot say before your appointment which test, if any, will be ordered. Questions about insurance coverage of genetic tests are best answered by the laboratory performing the testing and by your insurance provider (once the appropriate test is determined). Upon request at your visit, we can provide you with contact information for the laboratory that will perform your test.
