The Spinal Cord Injury Fellowship is a collaboration between Baylor College of Medicine and UT Health McGovern Medical School so it is only necessary to send one application to the program of your choice. Program coordinators will share your completed application with the other program in preparation for interview season. If you have a preference, please communicate it with a coordinator when submitting your application. Obtain a copy of the application. The form and all documentation is due no later than Aug. 15

  • Submit a Completed House Staff Application Form (available upon request)
  • Three Letters of Recommendation (including one from your present and/or past residency program director)
  • Certified Medical School Transcript
  • Curriculum Vitae (CV)
  • Certified USMLE Transcript
  • One Passport Size Photograph
  • Copy of Internship Training Certificate
  • Copy of Residency Training Certificate, If Applicable
  • One Page Personal Statement That Describes Your Specific Interest and Your Future Plans in This Area
  • OPTIONAL: Any relevant documentation that you feel will enhance your completed application.
