If you are under 18, Texas state law requires parental consent and also requires the parent to view the Vaccine Information Sheets and sign an acknowledgment.

  • HPV (3-dose vaccination; works to prevent the common types of HPV which can cause cancer)
  • Hepatitis B (helps protect against a virus that damages the liver)
  • PV (Polio vaccination: protects against nerve damage, which may lead to paralysis)
  • MMR (works against measles, mumps, and rubella)
  • TD (boosts immunity to diphtheria and tetanus)
  • Tdap (boosts immunity to diphtheria, tetanus, and pertussis (whooping cough)
  • Varicella (protects against chickenpox)
  • Hepatitis A (helps protect against inflammation of the liver)
  • MCV 4 (helps protect against meningococcal disease like meningitis or sepsis (bloodstream infection))
  • PPD (tuberculosis skin test: requires two visits to the doctor. In the first visit, you receive a shot. In the second visit, the doctor determines if you have an immune response to the test and can diagnose tuberculosis).

All vaccines are administered at our School-Based Clinic locations (Milby, Tejano, Wisdom, and Worthing).

Meningitis and HPV vaccines are administered at all Teen Health Clinic locations.

Consent forms are required for all services for minors under the age of 18 years old, except for STI screenings and pregnancy tests.
