Family Medicine Forms

Save time by filling out forms before you arrive. Each form can be printed for completion.

New patient forms. The General Intake Form can be filled out and submitted online if you have a MyChart account. If you have any questions regarding new patient forms, please call 713-798-7700.

Appointments

Content

Most appointments can be made within 24 to 48 hours. We offer same-day visits in some cases.

New Patients
The day of your first visit, please arrive 30 minutes before your scheduled appointment. Bring your current insurance card and a driver's license or government-issued photo identification. Medicare/Medicaid patients need to have a valid card and letter, as well as secondary insurance information. Please note your co-payment will be due at the time of service.

Returning Patients
Please arrive 5–10 minutes before your scheduled appointment. Be sure to bring your current insurance card and a driver's license or photo identification to each appointment. Please notify the front desk of any insurance, phone number, or address changes.

Cancellation/No Show Policy
If you cannot keep your scheduled appointment, we request that you notify the clinic as soon as possible. Appointments are in high demand. Your cancellation allows us to serve patients who may not otherwise have been seen. Thank you for your thoughtfulness.

May I bring my children to my appointment?
There is a children's play area available while you are waiting for your appointment. Children cannot be left unattended in the lobby during your appointment.

Prescriptions

How do I get refills for my prescription?
Please contact your local pharmacy with your prescription request at least three to four days in advance. They will contact our office for authorization. You may also contact your provider via MyChart for prescription questions.

Specialist Referrals

How do I get a referral to see a specialist?
If your health plan requires referral authorization, it is your responsibility to contact Family Medicine prior to making an appointment with a specialist. Failure to do so could result in lack of authorization for the specialist visit, which may cause your insurance company to deny payment. Retroactive referral authorizations will not be made. Generally, patients need to be seen by their primary care physician before a referral can be requested.

You may request a referral online if you are a registered patient by logging in to MyChart.

By phone
Call 713-798-7700. Please provide the name of the Family Medicine physician requesting the referral, the patient's name, birth date, phone number, and indicate if the referral is new or a follow-up. Personnel from the specialist's office should fax orders or notes to 713-798-7775.

How long will it take to obtain a referral?
Initial referrals usually take three to seven business days to complete. Follow-up referrals typically take three to five business days.

Test Records

When will my test results be available?

Most test results are available immediately when records are finalized by the lab, depending on the kind of test you have performed. Your provider will release test results to you through MyChart or a phone call.

If you have not received results two weeks after your test, please call Family Medicine at 713-798-7700. Please provide the patient’s name, birth date, phone number, and the specific test results you are inquiring about, as well as the date and location of the test/service. Calls are typically returned by the next business day.

How can I get a copy of my medical records from Family Medicine?
A completed and signed Authorization for Release of Protected Health Information form can be sent to our Release of Medical Records department as follows:
