Transition Medicine is a unique Section within the Department of Medicine. The team provides healthcare and social support services to young adults and adults with developmental disabilities who have been receiving care from Texas Children's Hospital and their affiliates. Many of the young adults/adults seen in our clinic have conditions such as Down syndrome, Spina Bifida, Autism, Cerebral Palsy, and other intellectual and developmental disabilities.

The Transition Medicine Clinic’s specially trained physicians and licensed medical social workers help these patients and their family access adult primary care, navigate the healthcare system, and identify and maintain the social services essential to their continued well-being. Our team creates long-term care plans for success and address current issues that adults with developmental disabilities face.

Baylor Transition Medicine Clinic has a health care transition peer mentorship program for individuals with intellectual and developmental disabilities. This program includes training on the mentor/mentee roles and responsibilities, professional boundaries, motivational interviewing, and goal setting.
