ACGME data indicates that despite knowing the clinical learning environment process for reporting adverse events, few trainees do. To improve our trainees' preparedness in providing safe care for their patients, we will:
- Provide housestaff with education in patient safety event reporting and disclosure of adverse events, and the opportunity to engage in an (interprofessional) real or simulated critical event review;
- Optimize tracking of error reports and improve feedback loop for communication following error reporting to housestaff in the clinical learning environment;
- Require completion of the IHI Open School Basic Certificate in Quality and Safety by all residents and fellows.
