The purpose of this study is to:
- Examine the potential of the use of POC HbA1c test to identify patients with pre-diabetes or newly diagnosed diabetes
- Evaluate the difference of HbA1c in patients diagnosed with pre-diabetes and diabetes that are enrolled in this initiative at 3 and 6 months.
